Birth Certificate

It is state law that we have a copy of each student's birth certificate in their cumulative file. The State Statute can be found here for your reference.

Immunization Records

A signed statement from health provider specifying immunization received, dates, and dosages is required for the following: poliomyelitis, diphtheria, pertussis, tetanus, measles, mumps, rubella and varicella.

If no, please note that non-immunized students are not permitted to attend schools unless 1) the Parent or Legal guardian provides an Immunization Exemption Form or 2) the Parent or Legal guardian provides a written statement that the Student will be immunized within 90 days of submission of the Student Enrollment Information form.
